Metformin, the active ingredient in Glumetza, selectively targets the liver to reduce hepatic glucose production and control hyperglycemia without significantly impacting insulin secretion. It effectively reduces both basal and postprandial plasma glucose levels. Its mechanism also includes slowing the intestinal absorption of glucose and improving insulin sensitivity by increasing peripheral glucose uptake and utilization.

Description

Glumetza is a brand name for an extended-release formulation of metformin hydrochloride, primarily used to improve glycemic control in adults with type 2 diabetes mellitus. As part of the biguanide class of medications, Glumetza works by decreasing glucose production in the liver, decreasing glucose absorption in the intestines, and improving insulin sensitivity by increasing peripheral glucose uptake and utilization. Unlike other diabetes medications, Glumetza does not increase the amount of insulin produced by the pancreas, making it less likely to cause hypoglycemia.

Additional information

Usage

Glumetza is indicated for:

● Improving glycemic control in adults with type 2 diabetes mellitus, in conjunction with diet and exercise.
● It may be used alone or with other antidiabetic medications.

Patients are advised to:

● Take Glumetza exactly as prescribed by their healthcare provider.
● Swallow the tablets whole; do not crush, chew, or break them.
● Continue their recommended diet and exercise regimen while on Glumetza.

Precautions:

● Glumetza carries a boxed warning for lactic acidosis, a rare but serious complication.
● Kidney function should be assessed before starting and periodically thereafter.
● Use with caution in patients with risk factors for lactic acidosis, such as renal impairment or conditions leading to hypoxemia.
● Consumption of alcohol should be limited due to an increased risk of lactic acidosis.

Side Effects

● Gastrointestinal symptoms such as diarrhea, nausea, vomiting, flatulence, indigestion, abdominal discomfort, and anorexia.
● Vitamin B12 absorption may decrease over time, potentially leading to deficiencies.

More serious but less common side effects requiring medical attention:

● Lactic acidosis, presenting as weakness, increasing sleepiness, slow heart rate, cold sensation, muscle pain, shortness of breath, stomach pain, feeling light-headed, and fainting.
● Hypoglycemia when used in combination with other antidiabetic medications.

Storage

● Store Glumetza at room temperature between 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F).
● Protect from moisture and light by keeping it in the original container with the lid tightly closed.
● Do not store it in the bathroom.

Special Precautions

Before using Glumetza, inform your healthcare provider if:

● You have a history of kidney or liver problems, heart disease, or are at risk for low vitamin B12 levels.
● You are taking other medications, including over-the-counter drugs and herbal supplements, as they may interact with Glumetza.
● You are pregnant, planning to become pregnant, or breastfeeding. The effects of Glumetza on the fetus or newborn are not fully known, and it may be necessary to adjust your treatment plan.

Overdose

Symptoms of a Glumetza overdose are consistent with its side effect profile, with lactic acidosis being a critical concern. Management should focus on supportive measures, including immediate medical attention, monitoring, and symptomatic care. In cases of significant overdose, hemodialysis may be required to remove metformin from the blood.
